1. Include your first name.
2. Day and time you would like to meet with Dr. Werner. Provide at least one alternative day and time.
3. Dr. Werner will respond to confirm your appointment.
4. You will be provided a Zoom link for the session after our appointment is confirmed.
5. I will email my Client Services Agreement to you which must be signed and returned prior to our initial session.
